This document should be placed in your copier to be copied. Place it face down with the circle and "supplement" indicating where the bottom of the page is. Next, put blank HICFA forms into the location where you would normally put blank paper. You should determine how to place the blank forms in this location prior to making copies. You can do this by putting an X on the top of a blank piece of paper and running a copy. That will indicate how your copier moves the blank paper through your copier to make a copy. This will indicate how you should place the blank HICFA forms into the copier. (Either face up or down and which end should be up or down)
